Use these forms to build your project/sub-award notification package, which should be emailed directly to your NETL Federal Project Officer.

ButtonNameResilience Project/Subaward Notification Submit this project notification template for each planned sub-award

ButtonNameSecretarial Eligible Entity Designation Request If applicable, use this template if your sub-awardee is not an eligible entity as defined in the program requirements. Reference Site for Utility Sales

ButtonNameEnvironmental Questionnaire (NEPA) Complete and submit this form with project specifics so the NETL Federal Project Officer can request a NEPA Categorical Exclusion (CX) determination.

  • Note 1: Adequate map(s) need to be provided along with the EQ; Google Earth is recommended in order to provide sufficient detail for project location(s).
  • Note 2: The DOE NEPA compliance team may require a permission letter from an appropriate agency overseeing proposed work on any national/state park or historic district lands. Access letters may be requested from the US Forest Service, State Parks Department, State Historic Preservation Officer (SHPO), Tribe’s THPO, etc.

ButtonNameCost Match Commitment Letter Statement from the eligible entity committing to meet the project cost match (100% or 1/3) as required in IIJA Section 40101(h). Include fiscal year, cost match dollar value, source, type, signature.

The Annual and Program Metrics and Impact Report is used by recipients to report on the impact of their projects.
